Documented incidents where an officer was involved in a death or serious injury but was cleared with no charges — by ASIRT, the Crown, or a fatality inquiry. In almost every case the officer returned to duty.
"Cleared" is not the same as "nothing happened." A no-charge decision means the relevant body concluded the threshold for a criminal charge wasn't met — often that the force was found "reasonable," or that the Crown saw no reasonable likelihood of conviction. Families frequently dispute these findings. Officers' return-to-duty status is rarely disclosed by ASIRT or in news coverage, so it is not shown per-record unless reported.
